For First-Time Visitors
This is the IRS-required intake form (Form 13614-C) that lets us prepare your taxes through the VITA program. It asks about your income, family, and tax situation—so we can get your return done right the first time.
For Returning Clients
Even if you’ve filed with us before, the IRS requires a new Form 13614-C each year. It helps us make sure your return is accurate and up to date.
For First-Time Visitors
This is the IRS-required Taxpayer Consent Form (Form 14446). It permits us to prepare your return using your documents—even if you're not with us in person. We use this form for virtual tax prep and also when we start your return in person, but need to finish it later without you at the tax center. It helps us serve you securely and efficiently.
For Returning Clients
Even if you’ve filed with us before, the IRS requires a new Form 14446 each year. It allows us to work on your return when you're not present—whether you're filing virtually or we need to finish your return after your in-person visit. This form helps us follow IRS rules and keep your information safe.
For First-Time Visitors
We ask all new clients to complete a short survey that helps CARE collect demographic information. This data supports our grant funding, which allows us to offer free tax preparation services. Your participation helps keep this program available to the community—thank you!
For Returning Clients
Even if you’ve completed this survey before, we ask that you fill it out again each year. It helps us meet grant requirements and continue offering this service at no cost. Your time makes a real difference—thank you!
For First-Time Visitors
This form gives us permission to use basic, non-identifiable information—like your age range, household size, or income bracket—from our CharityTracker system. This helps us apply for grants and report on the impact of our services. It does not include your name, address, or contact details. It also allows more than one IRS-certified volunteer to help prepare your return, so we can serve you more efficiently.
